Do not submit patient names, dates of birth, diagnoses, insurance details, clinical notes, or other protected health information through this public webpage or ordinary email.
For referral coordination, call the clinic or fax referral documentation using the fax number listed on our locations page. Ask the office to confirm its currently approved secure intake method before transmitting records.
